HSBC Credit Protector and Credit Protect Plus Claim Requirement
CLAIMS REQUIRMENTS
I. FOR DEATH CLAIMS
Primary Documents
i. If death is due to accident / violent cause
a. Certified True Copy of Death Certificate
b. Claim Form (please refer to “CLAIMANT STATEMENT–Death Claim” Form). To be accomplished by the immediate family.
c. Certified True Copy of Final NBI or Police Report
d. Certificate of Outstanding Balance(c/o HSBC)
e. Recent Credit Card Statement of Account (c/o HSBC)
ii. If death is due to causes other than accident
a. Certified True Copy of Death Certificate
b. “ATTENDING PHYSICIAN’S STATEMENT-Death Claim” Form
c. Certificate of Outstanding Balance (c/o HSBC)
d. Recent Credit Card Statement of Account (c/o HSBC)
iii. Death Abroad – Documents pertaining to death of insured abroad must be duly authenticated translated/verified by the Philippine Embassy of the country where death occurred.
Secondary Documents- to be required only if the information on the Death Certificate, Claim Form or Attending Physician’s Statement is insufficient to complete an evaluation of the claim.
a. Certified True Copy of Admitting or Medical History
b. Certified True Copy of Clinical Abstract
c. Certified True Copy of Autopsy Report, if applicable
d. Criminal Complaint, if applicable
e. Statement of Identifying Witnesses, if applicable
f. Certificate of Outstanding Balance (c/o HSBC)
g. Recent Credit Card Statement of Account (c/o HSBC)
II. FOR PERMANENT DISABILITY OR ACCIDENTAL DISABLEMENT
Primary Documents
a. “CLAIMANT STATEMENT-Disability” Form
b. “ATTENDING PHYSICIAN’S STATEMENT-Disability Claim” Form
c. Certificate of Employment stating dates on sick leave without pay
d. SSS Certification for Total & Permanent Disablement claim, if any.
e. If hospitalized, Certified True Copy of Admitting History, Medical History or clinical abstract stating insured’s condition and the
activities of daily living that insured can perform.
f. Notarized statement of tasks performed at work before and after diagnosis/disability.
g. X-ray or other diagnostic examination - in the event of partial loss of any part of the body specified.
h. Certified True Copy of Police Report for accidental cause.
i. Certificate of Outstanding Balance (c/o HSBC)
j. Recent Credit Card Statement of Account (c/o HSBC)
III. FOR LIFE EVENTS CLAIMS (For Credit Protector Plus Only)
i. Lost Card Protection (TAT 45 days)
a. Letter of advice about the fraudulent transactions (dispute letter)
b. Other documents as maybe required by the claim’s adjuster (disputed transactions slip)
c. Copy of valid ID
d. Dispute Resolution Form (HSBC)
ii. Life Events Benefit
a. Involuntary Unemployment
o Certificate from employer (length of employment; date and reason of termination)
b. Hospitalization Claim
o Hospital billing statement
o Copy of Attending Physician’s Discharge Order
c. Legal Separation Claim
o Certificate copy of Court Order
d. Dwellings (Calamities Claim)
o Certification from Barangay Office
o Photos of the damages incurred
e. Bereavement Claim
o Death Certificate of the deceased (Immediate family member)
o Policyholder’s Birth Certificate (if deceased family member is policyholder’s parent or child).
o Marriage Contract (if deceased family member is policyholder’s spouse)
