Applying for life insurance

Policy contract

To request for a replacement contract, please submit to your agent or any Pru Life UK customer center near you a duly accomplished and notarized Agreement Pertaining to Loss or Destruction of Policy Form along with payment of PhP150.00. You may download the form here.

Your policy contract can be used as part of a collateral agreement in which the proceeds of your policy are shared with your assignee (the individual receiving full or part of your insurance proceeds). To assign your policy contract, submit it to your agent or any Pru Life UK customer center near you together with a duly accomplished and notarized Assignment of Policy Form, a copy of the  Loan Agreement Form and consent of Irrevocable Beneficiaries and Release of Previous Assignment Form, if any. As soon as you have fully paid your loan from where you assigned the policy, you may request for the release of your policy's assignment. To do this, submit to your agent or any Pru Life UK customer center near you the following: accomplished Release of Policy Assignment Form, Policy Amendment Request Form (for change of beneficiary), and Secretary's Certificate of Board Resolution (if the Assignee is a Corporation) authorizing an officer to sign for the Company. Policy transactions

You may update your contact information online through PRUAccess, our online customer portal where you can access your policy information 24/7. You may access PRUAccess here.

Alternatively, you may also accomplish the Contact Information Update Form and have your agent process it for you, or you may directly submit it to your agent or any Pru Life UK customer center near you. You may download the form here.

Personal information includes correction of the Insured or policy owner’s name, change in civil status, occupation, address, contact numbers and date of birth without change in age.

You may update your personal information online through PRUAccess, our online customer portal where you can access your policy information 24/7. You may access PRUAccess here.

Alternatively, you may submit a duly accomplished and signed Policy Amendment Request Form, along with necessary documents such as birth certificate, marriage contract, specimen signature card, and a valid government-issued ID. You may change your beneficiary/ies online through PRUaccess, our online customer portal where you can access your policy information 24/7. You may access PRUAccess here.

Alternatively, you may also submit to your agent or any Pru Life UK customer center near you a duly accomplished and signed Policy Amendment Request Form with consent of the irrevocable beneficiary/ies, if any. You may download the form here.

Should you as the original policy owner surrender your rights over the policy to another person with exactly the same insurable interest to the insured, the new owner is required to submit a duly accomplished and signed Policy Amendment Request Form along with a valid government-issued ID, policy contract, and a specimen signature card. As the original owner, you also need to sign the form. You may download the form here.

For traditional plans, you may request to change your chosen non-forfeiture option and dividend option anytime. To do so, simply submit a duly accomplished and signed Policy Amendment Request Form to your agent or any Pru Life UK customer center near you. You may download the form here.

You may add an optional benefit to your policy any time to make your policy fit your needs better. This requires paying an additional regular premium that corresponds to the sum assured of your chosen optional benefit. To make this request, simply submit a duly accomplished and signed Policy Amendment Request Form (PARF) and Declaration of Proof of Insurability to your agent or any Pru Life UK customer center near you. You may increase the sum assured of your policy once every policy year up to the maximum sum assured amount allowable for your current age, and as long as you are under age 60. To increase your policy’s sum assured, simply submit a duly accomplished and signed Policy Amendment Request Form and Health Statement Form to your agent or any Pru Life UK customer center near you. Additional requirements may be required depending on the assessment of your request. You may download the forms here

You may reduce the sum assured of your policy after the first policy year and thereafter, once every policy year. To make this request, please submit to your agent or any Pru Life UK customer center near you a duly accomplished and signed Policy Amendment Request Form. You may download the form here.

Yes, you may switch the units of your fund to other funds that are available on your plan. You may apply for a fund switch online through PRUaccess, our online customer portal where you can access your policy information 24/7. You may access PRUAccess here.

Alternatively, you may submit a duly accomplished and signed Premium Redirection/Fund Switch Form along with the letter of consent of your irrevocable beneficiary/ies or assignee (if applicable) to your agent or to any Pru Life UK customer center near you. You may download the form here.

You can make an additional one-off payment (“top-up”) to increase the units invested in your fund, subject to the Company’s rules and regulations, and if the person insured is within the acceptable age limits. To top up, simply submit a duly accomplished and signed Top-up Form, Personal Financial Statement and Agent’s Confidential Information, along with a valid government-issued ID to your agent or any Pru Life UK customer center near you. You may download the forms here.

Depending on your plan, you may increase or decrease the amount of your regular premium. 

You may increase the amount of your regular premium at any time as long as the person insured is under age 60. To request for an adjustment in your regular premium, simply submit to your agent or any Pru Life UK customer center near you a duly accomplished and signed Policy Amendment Request Form (PARF). 

You may also reduce the amount you pay as long as it is equal to or more than the minimum amount allowable for your plan. To request for an adjustment on your regular premium, please submit to your agent or any Pru Life UK customer center near you a duly accomplished and signed PARF.

Once your policy has a withdrawal value, you can apply for a partial withdrawal at any time by asking us to sell some of the units allocated to your account. The price used to sell units depends on the timing when we receive your application.

If you partially withdraw from your policy/ies, you will reduce its fund value and death benefit in the event of a claim. The fund value may also become insufficient to support your insurance coverage and its charges. In this instance, it might be necessary to provide additional premiums and top-ups in the future to ensure that your policy remains active. 

To request for a partial withdrawal, submit a duly accomplished and signed PRUlink Application for Withdrawal Form with consent of irrevocable beneficiary/ies, if any, and a valid government-issued ID to your agent or any Pru Life UK customercenter near you. You may download the form here.

Paying for your policy premiums

We encourage you to pay your renewal premium on or before the specified due date to ensure that your policy/ies remain in force. Should you miss your due date, you can still pay your premium within the 31-day grace period without incurring interest.

For traditional plans – If you fail to pay for your premium before the end of the grace period, which is 31 days after the due date, your policy will lapse. However, if your policy has already earned sufficient cash value, the Company will apply your chosen Non-Forfeiture Option (NFO). This is a provision that allows a policy with enough cash value to continue coverage under specific circumstances, even if a renewal premium is not paid by the end of the grace period. 

For regular premium unit-linked plans (insuravest) - If the premium payment is not received before the end of the grace period or 31 days after the due date, the policy will lapse if its fund value is insufficient to settle insurance charges. However, if the policy already has a sufficient fund value, Pru Life UK will deduct the insurance charges from the fund, through unit cancellation, in order to keep the policy in force. The policy will lapse once the fund is no longer sufficient to settle insurance charges.

We encourage you to get in touch with your agent to have them process the policy reinstatement in your behalf. You may also call our PRU Customer Action Team to check whether your policy is still qualified for reinstatement through [email protected] or  (02) 887 5433 (Metro Manila); 1 800 10 7785465 (domestic toll-free for PLDT landline) from 8:30 a.m. to 6:00 PM, Mondays thru Fridays except holidays.

After your payment has been posted on our system, a copy of the Official Receipt will be delivered to your nominated address within 2-3 working days. The receipt can also be accessed online through PRUAccess.

While we cannot issue another copy of your official receipt, we can provide you with a certificate of payment. You may also view copies of your receipts online through PRUAccess. The receipts are available one year back from date of access.  

Online receipts on PRUAccess only go back as far as 1 year from date of access. If you wish to get a certificate of payment for older receipts, kindly contact our PRU Customer Action Team via phone at (02) 887 5433 for Metro Manila, 1 800 10 778 5465 for PLDT landline from 8:30 a.m. to 6:00 PM, Mondays thru Fridays except holidays, or via email at [email protected]

All requests for change in your premium payment schedule are restricted to logical dates and should coincide with your policy’s anniversary or due date. Request for change to a more frequent mode of payment (e.g., annual to quarterly) will take effect on the next premium renewal date. Claims

The requirements for filing a claim depend on the claim type. Please get in touch with the policyowner's agent to assist with the filing of your claim.
